Trying to put the dock plate back on the trailer after delivery

EE was attempting to pick up a dock plate wedged between the truck and the wall of the dock at the store (Big Bunny Southbridge [REDACTED]). EE felt a sharp pain in his left lower back and the pain travelled down his left leg glute. EE tried to stretch it out but pain and tightness increased as time went on. Upon return from first trip he contacted MOD to report the incident and will be seen at AFC Marlboro.
