Flattening a Bent [REDACTED]
What happened
EE received 1 inch laceration while attempting to flatten a bent anneal spacer using the hydraulic flat bar machine. EE was holding spacer with left hand thumb on top and left index finger wrapped under the back end of the spacer. Tip of left index finger was pinched between the base plate and spacer. When the bend in spacer was put under pressure the spacer ends flexed down pinching finger See attached photo. EE treated at [REDACTED] ER received 6 sutures to close laceration x-rays negative. No lost time EE released to return to work. Post incident substance abuse test result was positive. EE employment terminated.
