Lifting HDPE board
What happened
On [REDACTED] at [REDACTED] a production worker in the flex pipe assembly area was standing next to the lift device at the point where the device arm is attached to the main frame. Employee and a colleague had loaded the 4x8 (weighing approximately 45 lbs) HDPE board on to the lifting arm she then grasped the arm of the fixture to rotate the board into the work position. The lifting arm then detached from the main frame which caused the now loaded arm to drop on to the employee 's left foot above the safety shoe protected area. The arm section of the device and the HDPE board made contact with the employee 's foot.
