Moving a crate onto a pallet

On [REDACTED] at approximately [REDACTED] an [REDACTED] Wax Clerk was assisting another employee with trying to put a crate with a die inside that weighed 170lbs on to a pallet to deliver to its department. Employees jacked up one side of the crate slid the pallet under the crate then simultaneously lifted one side of the crate to fully place it on pallet. Employees were shifting the crate to get it in the center of the pallet when the [REDACTED] Wax Clerk felt a pain in his right tricep.
