What happened

On [REDACTED] at about [REDACTED] a knockout operator went to remove castings from a tumble blaster. He did not have molds to knock out which is his usual position. Being that there was no molds to knock out he was blasting revert using the tumble blast machine. There are two screws that are what holds the door into place one set of screws are the screws that the operator adjust to remove the door the top screws were not secure and when [REDACTED] rotated the tumble blast to set the door into position the door fell off as the tumble blast rotated. Once the door fell off of the hinges that hold it in place the door came down on [REDACTED] feet. Above the steel toe part of his boot. Around the shoe lace part. After the incident [REDACTED] went to break and once he was at break he let [REDACTED] know if what had occurred. [REDACTED] advised [REDACTED] to let [REDACTED] know.
