Cleaning Slurry Pot
What happened
On [REDACTED] at approximately [REDACTED] a Shell Operator was cleaning the DS Pot in the Shell Mixing Area. He had cleaned the sides of the pot and pumped out the residual slurry. He then tilted the mixer back to bring the propeller up closer to the top of the pot. He tightened the clamp to hold the mixer in position and began rinsing the propeller blade with the hose directing the spray into the slurry pot. While rinsing the blade with the hose the clamp failed to hold the mixer in place. The weight of the motor pivoted the propeller blade up striking the operator on the right ring finger causing a laceration.
