What happened
On [REDACTED] at approximately [REDACTED] the Phoenix Team Lead was unloading N Ray shipments of 15 boxes from the truck then unpacking the parts in the boxes. Each box had anywhere from 1 piece to 24 pieces. While unpacking the last tote the employee reported that his back started hurting and his right leg started going numb. This was initially filed as a First Aid. As the pain and occasional numbness has not improved the employee elected to go to Concentra for treatment. He visited Concentra on [REDACTED] and was prescribed medication along with lifting restrictions.
