Before the incident
On [REDACTED] the 3rd shift [REDACTED] Lathe operator encountered a machine malfunction. The electric eye that detects the next tube on the belt failed to sense the next rubber tube in line causing the belt to continue to advance and two tubes dropped one of them was turned vertically. The rubber tubes typically fall horizontally on to the cradle parallel to the belt which advances the next tube to the mandrel. The operator reached into the machine to remove the second rubber tube and simultaneously the conveyor advanced causing the operators left middle fingertip to get caught between the raised cleat on the conveyor belt and the aluminum ramp that the rubber tube rolls down towards the cradle.
