Ironworker was being trained on how to use the HC 285 [REDACTED] Representative.
What happened
As Operator stepped down onto the ladder his left foot slipped due to condensation on the ladder rungs which caused his left foot to go in between the ladder rungs causing him to lose his balance and fall. Since his left foot was inside the rung of the ladder his body rotated and that is when his left leg fractured due to his foot being caught inside the rungs. Operator fell approximately 8ft to the ground below.
