Before the incident
Loading A frame from the trim table in the lamination department
What happened
Two Team members were loading A-frame from the trim table in the lamination department. While loading the A-frame the one team member that was walking backwards slipped on a mat that is used for cushioning while standing at the trim table to trim. This caused the team member to fall to the ground while holding the glass. Other team members in the area quickly took the glass from the team member that had fallen. After a few moments of rest team member developed a slight pain in his lower back area. Team member refused EFR treatment. Sending the team member over to physical therapy for further evaluation. Video confirms the TM was walking backward with another TM when he tripped.
