Before the incident
He was l was trying to get a finished bale of tires out of the baler. The eject plate was again not functioning as the chain was broken. This happens frequently. [REDACTED] had his forks under the bale and was atempting to lift it up to get it out of the baler and his back tires lifted off the ground because the bale is very tight in the baler. He jiggled the lift lever attempting to break the bale free and also backed out trying to bring the bale out of the baler. His forks came out from underneath the bale while his back wheels were off the ground and the forklift instantly dropped back down on all 4 wheels jarring him in his seat and causing pain in his neck and back. I didn 't get a statement from [REDACTED] as he was taken by ambulance but according to a witness ([REDACTED]) he stayed sitting on the truck for a few minutes and then got off and sat down in a chair. [REDACTED] then called me and I responded. The Plant Coordinator dispatched the Goodyear nurse and she then called for
