[REDACTED] the extrusion operator was setting up his equipment by placing a PVC pipe onto the shaft. After positioning the pipe he attached the chuck to the shaft to hold it securely. Using an Allen wrench he began tightening the chuck. Unfortunately he applied too much force causing the wrench to snap. A piece of the broken Allen wrench remained lodged in the chuck while the other piece remained in his right hand. The piece that he was holding in his hand grazed his left forearm causing a 5 inch cut.
