On [REDACTED] a material handler reported that he had some lower back pain that was causing some numbness and tingling in his feet. The employee could not pinpoint a specific injury; however believed it to be associated with material handling duties. Employee said he had been icing and taking OTC ibuprofen at home for the [REDACTED] since starting the position to help with normal work conditioning. However the pain he felt [REDACTED] was significantly higher. OrthoLive was utilized and conservative management with a current stretch-and-flex program. On [REDACTED] [REDACTED] sent EE for X-ray. On [REDACTED] 25 OrthroLive sent the EE for an MRI on [REDACTED]. The employee agreed to go to Occupational Health for an Evaluation. Dr [REDACTED] Placed EE on Restricted Duty Following Examination referred to Orthopedics
