Before the incident
Operator had to step in close to foot rail to get strand behind the breakout on #38 and ends out on #39. As she moved towards chopper her left foot caught against the foot rail causing her to fall unimpeded.
What happened
Operator was gathering filaments on 525 pos. 38 after a breakout. Pos. 39 had some ends hanging out of the right side of the bushing as well. Another operator further down was taking a strand to the chopper. As he approached the operator at pos. 38 he handed the strand off to her to take to the chopper. As she proceeded she bent in to take the strand behind the breakout on 38 and the strands out on 39. As she stepped toward the chopper her left foot caught the foot rail on the grating causing her to fall. She hit her face on the chopper though it is unclear which part of the chopper she hit her face on. (We viewed the fall on video but angle prevented us from seeing what her face made contact with). She also fell hard on her right arm and knee. Another employee saw her fall and went to help her up from the floor. A different operator called the Area Manager and T&M Manager who is an EMT. They both immediately went to the injured employee. After assessing employee EMT aided her down the
