DESCRIPTION: On [REDACTED] at [REDACTED] An employee leaving their shift slipped and fell on an icy untreated section of the parking lot. Snowfall and low temperatures had rendered salt ineffective and the area remained unplowed as the operator was unavailable [REDACTED]. While a cleared and salted path was accessible the employee chose to walk into the snow-covered manager 's parking section to remote start their vehicle. They slipped on the same icy patch twice the second time while conversing with a colleague. ROOT CAUSE(S): 1) Insufficient planning and resource allocation for snow and ice management. The current process relied on a single plow operator whose absence left areas untreated.
