DESCRIPTIONA Tool & Die employee was reinstalling the cut off die into the unit on Extrusion Line by pushing it with left hand. Employee stated that 'something didn 'tfeel right ' and when theypulled their hand outthey saw thecut. Employee went back to theTool & Die shopto wash the wound and it started bleeding. Two other employees later checked the unit and found no blood and no sharp surface to cause the laceration. ROOT CAUSE1. PPE (Cut gloves) was not required for the job. 2. Unknown object caused the laceration. 3. Cut gloves not available on site in the proper size for the injured employee. CORRECTIVE ACTIONS1.
