SITE CODE: 007 SITE: [REDACTED]. BODY PART: Left Shoulder - Left Hip. INJURY TYPE: Contusions to Shoulder & Hip. EMPLOYEE TYPE: Permanent. DEPARTMENT: Extrusion. RECORDABILITY RATIONALE: Rx & 3 lost workdays. DESCRIPTION: [REDACTED] @ approximately 1435 hrs employee had walked around the end of Extrusion Line 23 to move a window rack that was located between lines 22 & 23. When employee was pushing the window rack their boot slipped causing them to fall on their left side. The employee 's left arm was extended at the time of the fall and after the fall the shoulder had no movement without significant pain and seemed to be 'uneven ' with the other shoulder. ROOT CAUSE: Damp Floor-Line 23 was scraping when the employee walked around the end and there was some water dripping off the material onto the floor. CORRECTIVE ACTIONS: TBD.
