Before the incident
ACL surgery graft harvest
What happened
ACL reconstruction surgery was being performed on patient . Quadriceps tendon was harvested successfully from patient by surgeon. Graft was taken by me to the back table to prepare for reimplantation after Arthrex tight rope device was sutured on o the end of the graft. In graft preparation suture with attached needle was passed through the graft multiple times to secure tightrope to the harvested tissue. On the final pass of tying the knot the needle attached to the end of the suture was passed through the graft tissue in order to bury the knot. Quad tendon grafts are tough and some effort was given to pass needle and inadvertently pricked my left index finger tip. I was double gloved. Immediately I let go of needle and graft and left it on the back table. I removed each glove and noted a small prick with blood present. I notified nurse circulator who initiated safety scoop coordinated source blood draw and necessary labs. She personally walked source blood labs to WC Lab on first flo
