What happened

On [REDACTED] [REDACTED] was doing a bed setup at a pt home after hours. The pt needed the bed setup in the upper level of the home which required lifting the bed up a flight of stairs and in the process of doing so she hurt her back. This was not reported at the time of injury. On [REDACTED] I noticed [REDACTED] was a bit off from her baseline and asked if she was alright. She stated that she was fine just a little out of it. Later that day she proceeded to let me know that she tweaked her lower back the prior day [REDACTED] and I asked if she need to go to the Dr which she declined and said she was fine. On [REDACTED] I did not hear from [REDACTED] regarding any pain or issues related to this. On [REDACTED] I was informed by an employee that [REDACTED] was struggling with back pain and I advised them to have [REDACTED] to head home as I was not in the branch.
