What happened
A worker was receiving and storing mro parts in kardex number one in maintenance. The kardex is a vertical high-density storage unit. While completing this task the worker observed the extractor pulling a tray from the storage location. It was also pulling the tray above it at the same time. To avoid the second tray from falling the worker attempted to pull the top tray back into place. Not thinking about where the hand placement was. The counterweight was moving upward as the tray was lowering. The counterweight caught the hand of the worker between the counterweight and the machine frame.
