What happened

Had this chronic shoulder pain that come and go but aggravated getting worst doing a heavy patient on [REDACTED] felt the worst pain never felt before while doing this patient I stopped doing the care rested and pain went away. Since the pain went away I didn 't file safety2sahare report. I ignored until pain recurred again but it will go after resting. I went to see provider [REDACTED] and referred me to PT started [REDACTED]. Still I continued working until [REDACTED] when I decided I need to take a time off from work to rest my shoulder for I felt the pain not only on my right shoulder but radiates to my arm and felt numbness on my upper back and arms [REDACTED] I reported to our boss [REDACTED] and [REDACTED] regarding my problem and they were very helpful got a call from employee health right away advised me what to do. I 've asked 2 weeks time off from work need to call provider again [REDACTED] to get medical certificate.
