What happened
At approximately [REDACTED] on [REDACTED] a team member tripped and fell onto their left knee. The team member asked their Team Leader to go to Physical Therapy for a sore leg and never reported the incident. PT reported back that the team member had tripped and fell onto their knee while working. The team member was walking around a welder and tripped on the bolt of a post that was protecting a 2-point welder machine. Investigation: Video footage shows the trip and fall at [REDACTED] on [REDACTED]. TM did not report until [REDACTED]. TMs reasoning was that it didnt hurt really bad until later. Thought it was no big deal. This is [REDACTED]. Failure to properly report a work-related injury accident in a timely manner
