What happened
On [REDACTED] at approximately [REDACTED] while working in the receiving department Outfitter [REDACTED] was loading ammo onto a cart. The cart fell over while she was loading the ammo and a box of ammo weighing approximately 30 pounds struck her right foot. Ms. [REDACTED] contacted her supervisor immediately after the accident. Supervisor [REDACTED] contacted me and informed me of the situation. Ms. [REDACTED] was taken to the first aid room and first aid was administered. [REDACTED] was contacted and recommended self-care. The Outfitter made a follow up call to [REDACTED] and requested to be seen by a doctor. [REDACTED] setup the appointment and the Outfitter returned with her doctor 's notes.
