What happened
On [REDACTED] at approximately [REDACTED] Outfitter [REDACTED] was working the Storage conveyor line. She picked up a pallet to move it and her left thumb got caught in the pallet while she was sitting the pallet down. [REDACTED] stated the palled folded her thumb back causing extreme pain. Myself and supervisor [REDACTED] escorted [REDACTED] to the first-aid room. First-aid was provided and CorVel was contacted. [REDACTED] recommended she be seen at urgent care. She was sent to Piedmont Urgent care. She provided a copy of her doctors notes.
