On [REDACTED] at approximately [REDACTED] Outfitter [REDACTED] was removing merchandise from a gaylord on the sort consolidation floor. [REDACTED] stated she grabbed a box of ammo from the bottom of the gaylord and when she lifted the ammo she felt a pop in her right shoulder causing immediate pain. [REDACTED] notified her supervisor immediately after the incident happened. First-aid was provided and [REDACTED] was contacted. The Outfitter was sent for treatment and provided a copy of her doctor 's notes.
