What happened

On [REDACTED] at approximately [REDACTED] hours Outfitter [REDACTED] notified Supervisor [REDACTED] that he had sustained an injury to his left eye. [REDACTED] informed [REDACTED] that at approximately [REDACTED] hours he was unloading cases from a trailer on [REDACTED] when he felt a foreign object in his eye. [REDACTED] stated that he immediately rinsed his eye in the restroom and was still experiencing irritation following the rinse but decided to continue working. Following his lunch break [REDACTED] reported the injury to [REDACTED]. [REDACTED] then took [REDACTED] to the eyewash station where the eye was flushed for approximately 10 minutes. Following the eye flush [REDACTED] was contacted and [REDACTED] was directed to seek further evaluation at Mountain West Medical Center. [REDACTED] left work following the incident and did not return prior to the end of his shift. Physicians instructions are pending.
