What happened

On [REDACTED] at approximately 0225 hours as I was returning from break Outfitter [REDACTED] notified me that she had sustained an injury. [REDACTED] reported that at approximately 0210 hours she was walking between two vehicles in the parking lot on her break when she slipped on some ice and fell to the ground. [REDACTED] stated that during the fall she caught herself with her right hand and her left leg bent backwards causing pain in her left knee and lateral portion of her calf. Supervisor [REDACTED] was notified of the incident and [REDACTED] was offered first-aid and the opportunity to contact [REDACTED] regarding the incident. [REDACTED] declined both stating that she just wanted to report the injury in the event the pain worsened. [REDACTED] then took me to the area where the injury took place and I observed that there was a large area of the asphalt that was covered in ice and the area was slick and slippery.
