What happened

On [REDACTED] at approximately [REDACTED] Outfitter [REDACTED] working as Equipment Operator was involved in an incident that occurred at [REDACTED]. The incident took place while the outfitter was doing put away and he stepped off the pallet back onto his stock picker he fell and to catch himself he used his left shoulder and it jerked his shoulder and he hit his head above his left eye on the machine side railing. He said he was fine at the moment. Gave him and ice pack and [REDACTED] checked his eyes. The incident was reported to [REDACTED] immediately after the outfitter hit his head and jerked his shoulder. Witnesses included None. [REDACTED] was not called and the outfitter chose to return to work. On [REDACTED] Outfitter went to the Mountain West Medical Center for further evaluation. Outfitter was diagnosed with possible soft tissue injury and was prescribed a pain killer. Outfitter was taken off work until [REDACTED] per doctor 's order. Outfitter is supposed to be re-evaluate
