On [REDACTED] at approximately [REDACTED] Outfitter [REDACTED] working as Equipment Operator was involved in an incident that occurred at [REDACTED]06 lower [REDACTED] by the tunnel. The incident took place while the outfitter was pulling 1 's. According to initial reports the incident involved [REDACTED] was pulling 1 's in [REDACTED] had to get another aisle that was busy so she backed up machine and felt a jerk causing her to run into the [REDACTED] and hit her head on the stock picker cage was noted that the pull back cause the guide wire to stop . As a result of the incident the outfitter sustained a bump on the right part of her forehead. Response at approximately [REDACTED] included she called [REDACTED] and nurse said to self-care and outfitter was given an ice pack for her head.
