What happened
On [REDACTED] at approximately [REDACTED] hours Outfitter [REDACTED] reported that she had sustained an injury while working on the chute at [REDACTED]. Supervisor [REDACTED] and I were notified and we arrived to investigate the incident. When we arrived we observed that [REDACTED] had her right pinky and ring fingers bandaged together. [REDACTED] informed us that she was attempting to clear the chute when her right pinky finger had been smashed between two boxes of ammo that had slid down the chute. [REDACTED] stated that she believed she had fractured the tip of her pinky finger.
