On [REDACTED] at approximately 1445 hours Outfitter [REDACTED] arrived at work to report that she had sustained an injury while working on the shipping dock the [REDACTED]. Safety and Security Manager [REDACTED] was notified and he arrived to assist with the report. [REDACTED] reported that on the evening of [REDACTED] she was assigned five shipping lanes. [REDACTED] stated that following her lunch break she was re-stacking long boxes that weighed approximately 35 lbs. on trailer 080 in a higher and tighter fashion. [REDACTED] stated that she spent approximately 10 minutes attempting to stack the last box when she lost her grip and attempted to throw the box up.
