On the above date time employee stated as they reached for their personal twoway radio that was on their forklift they felt a sting bite on their left ring finger Employee immediately pulled their hand back and noticed some swelling Employee reported incident to safety at which time first aid treatment was administered in the form of an ice back antibiotic ointment as well as a band aid this did not aid employee The decision was made via Remote Athletic Trainer and Travelers Insurance to send the employee to be checked out at the Concentra Urgent Care Clinic Updates to follow report Attached to report are employees Work Activity Status Report and Transitional Duty Description please see attached reports for further details Employee is doing much better and their work restrictions have been removed employee allowed to return to full work duty Further details in latest WASR if needed any future details will follow Upon follow up with doctor employees ring finger still had the lump from t
