On [REDACTED] at [REDACTED] a production employee reported to Safety that their left shoulder has been hurting them since [REDACTED] Safety asked employee why was this not reported the same day the incident occurred Employee stated that she did not feel comfortable reporting incident to her supervisor Safety than proceeded to ask employee how did they get injured Employee stated on [REDACTED] they were assigned to work in Thermo Fishure for the entirety of their shift at which time while they were being rotated between two job tasks: building box inerds and placing bottles into their cases Employees states while working these twojob task that they felt pain in their left arm shoulder Employee was referred to remote athletic trainer for further evaluation and first aid treatment After weeks of first aid treatment with our remote athletic trainer employee had to go to the [REDACTED] for two weeks for personal a matter Upon return employee stated they no longer wanted to see the remote at
