Before the incident
Employee was making an inspection of the ammonia pipes valve on top of the icehouse roof.
What happened
Employee was inspecting the valve of the ammonia pipes on the top of the icehouse. Visibility of the valve and labeling of the pipe was restricted forcing employee to walk on the metal diagonal surface of the roof (platform). Employee slips and fall onto the metal roof of the icehouse causing a fracture of the left side of the hip. Ambulance was called and taken to the hospital.
