On [REDACTED] before shift at [REDACTED] EE stated to supervisor and safety manager that they began to feel back pain the [REDACTED] that had not dissipated. EE stated they could go on route and was advised to see [REDACTED]. On [REDACTED] EE came to Safety Manager stating they had back pain that started on [REDACTED]. EE was not able to provide a mechanism of injury but believed it was work related. After discussion about when the pain started it was determined EE would see PCP to determined work relatedness. EE returned stating it was work related and EE requested to be seen at the clinic. At the clinic EE received restrictions making it an MDI.
