The employee was cleaning scrap and shavings from the [REDACTED] machine.

While cleaning scrap from [REDACTED] the employee bypassed the machine guard and did not perform Lockout Tagout (LOTO). He inserted his fingers into the opening where the saw retracts to its home position in order to push shavings down. During this action his left index finger came into contact with the saw blade resulting in a laceration.
