Employee was adjusting top cover of washer 2
What happened
The employee was tasked with fixing a top cover on washer 2. To be able to reach the top cover the employee climbed up the guard rail and climbed onto the conveyor on their hands and knees. There is about a 12” gap between the work platform and the conveyor. So the employee was unable to access the top cover standing on the platform and there is no access on the other side. After they fixed the top cover as they were climbing down the employee stepped on the middle rail and slipped falling on their right knee.
