Upon receiving the initial report [REDACTED] instructed [REDACTED] to have the employee evaluated and treated by the onsite EMT.Medical Information:At [REDACTED] [REDACTED] contacted [REDACTED] again to report that the onsite EMT [REDACTED] had activated [REDACTED] requesting ambulance transport for the employee to the Endeavor Health Evanston Emergency Room.
