Before the incident

Installing a choker around the bottom of a wooden crate.

What happened

Employees were tasked with unloading a wooden crate from a Shipping Container. The crate involved was constructed with plywood with 4x4 Timbers attached to the bottom of the crate to allow for lifting forks to access the bottom of the crate. The crate was oriented in the Container perpendicular to the floor of the container. The employees attached a choker around the timber and back to itself. Ultimately they linked it back to the mass of a Rough Terrain forklift to pull the crate outward. The crate 's location in the container prevented full access to the crate. The employees intended to pull the crate outward enough to get under the crate with a pallet jack. Once the rigging was attached to the crate the injured employee waited as the Operator scoped in the boom tightening the choker. The IE then began to walk away from the crate and gave the operator the signal to continue scoping in. At this time a separate employee performing signaling to the forklift driver also gave the go ahead
