TM stated they were lifting heavy liquids at location [REDACTED] at approximately [REDACTED]. During the lift TM 's foot slipped on the edge of the machine and lost balance. During that the TM twisted lower back and felt pain instantly. TM reported the incident to the supervisor immediately but thought he was ok. TM felt pain later in the shift and left to go to their primary care. TM stated they did not know they needed to fill out a report or be seen by approved doctor through CorVel.
