What happened

Employee was performing a skid Load Test implementing a new process at UTB. While this process is being developed and compiled in a work instruction the employee was using a torque wrench with a torque multiplier to determine if the skid lifting points are secure. After setting up the equipment the employee was attempting to apply 50 foot pounds when a pop sound from the equipment initiated a failure of the set up and the system gave while the employee was applying pressure and with the release the employee 's knee hit the factory floor. The pop may have occurred for several reasons Torque wrench may have broke Turque multiplier may have either broke or certain pin may not have been fully engaged. Or whole system set up may not have been stable. Injury was not severe enough at time of reporting to warrant stopping(Employee 's words). Employee is continuing work and developing a safe system and work instruction .
