What happened

While in the process of normal manual lathe machining employee sitting down going over the part print employee was machining got up to proceed with employee 's work it was at this time standing up that employee experienced a sudden severe and seemingly debilitating back pain rendering employee unable to stand on employee 's own. At which point employee called for help from a second employee. Supervisor was brought onto the scene a minute or two after that. After some deliberations 2 employees and supervisor slowly placed 1st employee in a wheeled office chair from the tool crib. Employee did not feel it necessary to call [REDACTED] so 2nd employee volunteered to drive 1st employee to a care center. With a second supervisor 's help two supervisors wheeled employee to the front employee entrance where 1st supervisor had 2nd employee waiting with 2nd employee 's parked vehicle. As employee could not stand or walk unadded 2nd supervisor volunteered to accompany 1st & 2nd employee to IMG. I
