What happened
Athlete was moving an empty cart through the mod to transition from put away to consolidation. The athlete had an incident involving the liftgate at [REDACTED] between zones 1 and 2 which resulted in a contusion to the athlete 's head. Based on the athlete 's statement the liftgate was latched and then came loose while pulling the cart through. The liftgate has been inspected by a member of the safety team and was found to be in working order. An attempt was made to recreate the incident based on athlete statement. We were not able to recreate the incident based on the athlete statement. The athlete had left early and therefore the incident will remain under investigation until we can clarify with the athlete.
