What happened
Mill 5 had crashed and operator was trying to remove the tube from the mandrel in the area of the bead hammer. Operator was not wearing cut resistant gloves which are required and when grabbing and pulling on the tube his hand slipped cutting his middle finger on his left hand. Operator was also in violation of not locking out machine while clearing a jam.
