Working at the bagger putting shippable items in bags for shipping.

Team: On [REDACTED] we had a safety incident. The athlete notified their lead and a manager around [REDACTED] [REDACTED] that as she was approaching [REDACTED] 6 she stepped on the safety mat which slid causing her to strain their back. Employee stated that they are doing fine but feeling a little pain. After reviewing the incident place it seems like that safety tape that used to hold the mat has worn out. Corrective action was taken and removed the mat from the workstation and recommended to revisit all workstations to ensure that all other mats at workstations are secured.
