[REDACTED] was attempting to unjam relocate the washer that was on the threaded rod which is attached to the tension arm on the uncoiler table. Upon noticing that this washer was out of place he attempted to fix the issue when the tension arm then swung forward (while the arm was running) as a result of added tension in the line. These adjustments to the threaded rod are made to reduce or increase the tension on the tension arm on the uncoiler table. This helps the speed of the table while the line is running.

The washer on the threaded rod became lodged within the threads and created a pinch point. Once the tension arm released the operator finger was within the pinch point area cutting employee 's finger.
