Employee was walking in the front door. Associate was arriving for their assigned shift.

It was raining in [REDACTED] the day of the injury. As the associate was entering the facility to start their shift. they stepped into the vestibule and slipped on a small section of flooring next to the front door. The associates left left slid forward which caused the right knee to make contact with the ground.

Employee slipped on a small section of uncovered concrete flooring with wet shoes and pant bottoms. This caused his right leg to slip forward and then his right knee to come in contact with floor.
