Before the incident
EE was changing a propane tank got the tank in the harness lifted it up and when the EE went to close the latch EE felt something in her arm pull causing a burning pain EE went to Athletic Trainer for treatement report attached [REDACTED] EE had been coming in since DOI to athletic trainer with first aid treatment and seemed to be getting better after strips were applied on [REDACTED] and [REDACTED] EE stated felt much better EE came to me on [REDACTED] and stated she thinks she needs to go to doctor as pain in her shoulder was still there and was keeping her up at night We had AT evaluate EE and agreed EE should be seen EE went to Occupational Health on [REDACTED] where doctor prescribed muscle relaxers sent for xrays and requested MRI be completed and placed on restrictive duty Incident to be edited to a recordable restricted duty MRI came back to show 2 small tears and surgery recommended 0292024 EE had surgery modified report from RD to LT next visit 219
