Victory Packaging driver states on the above date and time they were using a tractor to relocate two trailers from [REDACTED] to [REDACTED] in the process of moving the two trailers the employee states the locking jaws which is called the fifth wheel was not disconnecting properly from the inside of the yard switcher controls Due to this mechanical issue the employee made the decision to step out of the switcher to manually loosen up the connection with the third wheel In completing this action said employee states he felt a slight pain in his right shoulder which at the time did not seem to bother them However employee stated that later in the night the pain reached a level of pain that caused him to go to the ER and seek medical attention This information was later given to the Dispatch Supervisor by the employee when they had time to do so Above stated employees incident statement is attached to this report Employee medically released on [REDACTED] and cleared for full work duty as
