On the above date and time employee stated while working USPS [REDACTED] they were packing several boxes ranging from 529 lbs onto multiple pallets throughout the day As the employees workday came near an end the employee felt pain in their midsection to lower right abdomen area which radiated to their lower back Employee stated they were not sure if this stomach pain and back pain were related to working in USPS [REDACTED] and that they were going to go see their own doctor Employee returned with a doctors and it was verified that all of their pain was stemming from a sciatic nerve in their back Being that this injury occurred on site while the employee was working the decision was made to send the employee to Concentra for further evaluation Employee returned from Concentra WASR Diagnosis Lumbar Strain return for followup 57 days Dispensed prescription medication for pain and modified work activity in attachments Physical Therapy dates pending Concentra will notify employee to set up
